Do I Qualify?
To receive a medical marijuana recommendation in Florida, you must:
Have permanent or temporary residency in the state of Florida
Be diagnosed by a certified physician with a qualifying condition
MD Marijuana Card Express also requires all patients to submit documentation confirming they have been previously diagnosed with the condition for which they are seeking medical cannabis treatment.

After you complete the form on this page, a patient care specialist will pre-screen you free of charge to determine if you may qualify for medical marijuana treatment in Florida. Once you have been pre-screened, we will schedule an appointment for you to visit one of our qualifying physicians at a convenient location and time. If our physician determines you qualify, they will enter you into the Medical Marijuana Use Registry, so the Florida Department of Health can issue you a certification.
